This page summarizes how we handle information collected through this website and how patient records are protected in our office. Information collected through this website
Our appointment request form collects only what our front desk needs in order to call you back and schedule a visit: your name, phone number, email address, whether you are an existing patient, an optional clinician preference, and your general availability. Please do not use the form to send detailed medical information.
Website form submissions are not part of your medical record and are not monitored outside office hours. Requests are handled by our scheduling staff and retained only as long as needed to complete scheduling.
Protected health information
Information in your medical record is used and disclosed for treatment, payment, and health care operations, and otherwise as permitted or required by law. Where authorization is required, we obtain it from you in writing before disclosure.
- Access to records is limited to staff involved in your care or billing.
- Records are maintained in an electronic health record with role-based access controls.
- Requests for copies of your records are handled in writing through the front desk.
