Your first visit

New patient visits are scheduled for extra time so your clinician can review your history in full. Please arrive about ten minutes early to complete intake paperwork.

  • Photo ID and your current insurance card
  • A list of medications, supplements, and doses
  • Names of any specialists involved in your care
  • Pharmacy name and location
  • Questions you want to cover — we'll make time for them

Insurance & billing

We participate in a range of commercial and Medicare plans. Networks change often, so please call the office with your plan name and member ID and we will verify your benefits before your appointment. If you do not have coverage, ask about our self-pay visit rates when you book.

Copays are collected at the time of service. Statements and payment questions can be directed to the front desk at (503) 274-8190.

Frequently asked questions

Are you accepting new patients?

Yes. We accept new patients of all ages and typically schedule a first visit within two to three weeks. Request an appointment online or call the office and our team will find a time that works.

How do I get a same-week appointment?

Reserve daily acute slots by calling the office in the morning. If all slots are full, our team will help you decide on the next appropriate option.

Which insurance plans do you accept?

We participate in a range of commercial and Medicare plans. Because networks change, please call the office with your plan name and member ID and we will confirm your coverage before your visit.

How do I request a prescription refill?

Ask your pharmacy to send a refill request electronically, or send a message through the patient portal. Allow two business days; some medications require an office visit first.

How do I get my test results?

Results are released to the patient portal as they are finalized, usually with a short note from your clinician. If a result needs discussion, we will call you.

What should I do in an emergency?

For a medical emergency, call 911 or go to the nearest emergency department. Our office is not an emergency or urgent care facility and phone messages are not monitored after hours.
